- Letter of Recommendation Request Form: Allied Health
All students seeking letters of recommendation for post-graduate education in any Allied Health Profession need to complete this form and submit it to the Allied Health Director no less than 30 days prior to the due date of the letter. Late requests may not be honored.
- Letter of Recommendation Request Form: Graduate School
All students seeking letters of recommendation for post-undergraduate education in any graduate program (master’s degree, Ph.D., law school) need to complete this form and submit it, along with an official copy of necessary standardized test scores (i.e., GRE, LSAT) and submit it to Dr. Marne Bailey no less than 30 days prior to the due date of the letter. Late requests may not be honored.
- Letter of Recommendation Request Form: Professional School
All students seeking letters of recommendation for post-undergraduate education in any professional degree program need to complete this form and submit it, along with an official copy of necessary standardized test scores (i.e., MCAT, OAT, PCAT, DAT, etc.) and submit it no less than 30 days prior to the due date of the letter. Late requests may not be honored.
- Guidelines for Applying to Professional/Graduate School
Students, please use these guidelines to assist you as you prepare for the application process. We suggest downloading a copy of these guidelines at least six months ahead of the application cycle, typically in January of the junior (or senior) year.

CASPer Testing
Developed by researchers in Ontario, Canada, CASPer is “an online screening tool designed to evaluate key personal and professional characteristics that make for successful students and graduates.” This tool has recently been implemented for use in several medical schools across the United States as another mechanism by which to evaluate potential admits. Currently, medical, optometry, pharmacy, veterinary, dental, and nursing programs in Canada are also using this tool.
